Hiring need: Care Ops/Care Navigator

Hours per week: 40-60

Expected Work Schedule: 6 days a week

Time zone needed: Central US Time

Role Overview: Patient Care Navigator

The Patient Care Navigator ensures a seamless and positive treatment journey for patients while optimizing clinician workflow and maintaining compliance. This role combines patient guidance, telehealth platform management, and customer service excellence with process development and quality assurance. Key responsibilities include:

  • Patient Care Navigation: Guide patients through their treatment, ensuring clarity and support at every step.
  • Triage & Workflow Optimization: Use sound judgment to triage patient inquiries, escalating clinician-critical concerns while managing routine issues independently.
  • Customer Service Excellence: Deliver detail-oriented, high-quality customer service, ensuring all interactions and documentation meet meticulous standards.
  • Telehealth Management: Efficiently oversee telehealth platforms for both asynchronous and synchronous communication.
  • SOP Development: Create, implement, and maintain SOPs to streamline customer care processes and improve efficiency.
  • HIPAA Compliance: Uphold rigorous standards of patient privacy and data handling in accordance with HIPAA.
  • Care Coordination: Act as a liaison among patients, healthcare providers, and stakeholders to ensure smooth communication and care delivery.
  • Continuous Improvement: Monitor service quality, identify areas for enhancement, and provide insights to drive operational improvements.

  • Fluent in English with excellent written and verbal communication skills.
  • Experience with EHR systems and telehealth platforms.
  • Previous experience as a Care Manager or Care Navigator.
  • Knowledge of HIPAA compliance requirements.
  • Reliable, high-speed internet connection.
  • Availability to work within a time zone compatible with the core team's hours.
  • Bachelor's degree in a relevant field (e.g., Healthcare Administration, Nursing, Public Health).

  • Experience working remotely.
  • Background in customer service or patient care within the healthcare industry.
  • Familiarity with mold-related illnesses or environmental health issues.
  • Experience in creating and implementing SOPs.
  • Certification in customer service management or healthcare administration.
