Job Summary

A company is looking for a Nurse Case Manager - Maternity.

Key Responsibilities

• Assess the needs of members and develop individualized care plans
• Monitor care plans and coordinate with providers for effective service delivery
• Educate members on their conditions, treatment plans, and available resources

Required Qualifications

• Registered Nurse (RN) with a current, unrestricted license
• Preferred college degree
• Three or more years of experience in a Case Management role
• Three or more years of clinical experience in OB/L&D or related area
• CCM certification required within specified timeframe
