Job Summary

A company is looking for a Care Manager (RN) to support their Ambetter Marketplace members remotely.

Key Responsibilities

• Evaluate member needs and barriers to care, recommending plans for optimal outcomes
• Develop and collaborate on ongoing care plans, ensuring access to necessary resources and services
• Monitor member progress and revise care plans as needed, providing education and support to members and families

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in case management, home health, or managed care
• Must hold a valid RN license, including compact state licensure
• Residency in North Carolina, Tennessee, or Georgia, working in the Eastern Time Zone
