Care Partners is seeking a detail-oriented and compassionate Eligibility Coordinator to support its growing healthcare brand in Orange County and the Inland Empire. We have recently launched a new service line specializing in care for children and dependent adults with developmental disabilities. We are a contracted provider with multiple Regional Centers throughout California, and we are looking for YOU to help us grow this exciting new platform!
The ideal candidate for Care Partners Eligibility Coordinator must radiate a passion for helping others and embody Care Partners’ core values of love, empathy, and compassion. The Eligibility Coordinator plays a critical role in verifying client eligibility, coordinating approvals, and ensuring seamless service initiation. This includes working closely with families, caregivers, and Regional Center representatives to facilitate access to needed services.
Requirements:
- Experience in eligibility coordination, case management, or a related role within the disability services field.
- Exceptional attention to detail and organizational skills.
- Strong interpersonal and communication skills, with a compassionate and professional demeanor.
- Proficiency in computer systems, including Microsoft Office, CRM platforms, and internet-based applications.
- A college degree is preferred but not required.
- Ability to pass a federal background screen.
- Valid driver’s license and proof of current auto insurance.
Essential Functions:
- Eligibility Verification: Serve as the primary contact for verifying client eligibility with Regional Centers.
- Review client applications, records, and documentation to ensure compliance with eligibility criteria.
- Communicate with families and Regional Center representatives to resolve any discrepancies.
- Documentation and Compliance: Maintain accurate records and documentation in our CRM system.
- Ensure all eligibility processes comply with state regulations and Regional Center requirements.
- Relationship Building: Develop and maintain positive relationships with clients, families, and Regional Center representatives.
- Act as a trusted resource for families navigating the eligibility process for disability services.
- Program Knowledge: Stay informed on policies, and service codes.
- Team Collaboration: Work closely with the Intake and Operations teams to ensure accurate eligibility determinations and smooth transitions to care services.
- Provide feedback and recommendations to improve the eligibility verification process.
