Washburn Center for Children is a leading children’s mental health center, caring for a wide variety of children’s needs associated with anxiety, depression, behavioral difficulties and trauma.

 

The 141-year-old nonprofit offers critical mental health intervention to help children - and their families - realize hope. Washburn Center for Children serves almost 4,000 children ages 0-18 and approximately 9,000 family members each year. Washburn Center’s three office locations – Brooklyn Park, Edina, and Minneapolis – serve the entire Twin Cities metro area. The nonprofit also offers in-home services, and its school-based program serves 50+ schools in the Eden Prairie, Bloomington, Minneapolis, Savage, Burnsville and Eagan school districts.

Washburn Center’s case managers work collaboratively with families and across multi disciplines internally, to develop a care plan and ensure access to needed services, including mental, social, educational, health, vocational and recreational assistance. Our case managers advocate on behalf of the child and family’s needs and coordinate care across multiple providers.  These services are provided for children age 3 through 17.

Washburn Center for Children is committed to all our employees’ professional development.  During your time at Washburn Center you will receive continuous clinical training, increased supervision time and exposure to a wide array of clinical needs.  Case Management is a unique field which will provide you with an expansive understanding of family mental health.  As such this role is well suited for Social Work experienced individuals or Mental Health qualified Professionals. Additionally this role is a highly collaborative function, and you will experience exposure to all programs at Washburn Center and work in collaboration with the various experienced professionals at Washburn to ensure the best care for the children and families in your care.

 

The case management professional will be actively present in the community and the home life of the client.  We encourage independent working for all our case managers to allow for the best client care and services. 

 

ROLE RESPONSIBILITIES:

1.       You will be managing a caseload of 15 - 17 children or youth and their families with all client service expectations that accompanies this caseload.

2.       At Washburn Center for Children we are committed to provide culturally based, individualized targeted Children’s Mental Health Case Management to children/students and families in their home, schools and community based on each family’s needs. 

3.       You will coordinate mental health and other health related services.  The purpose of this involvement in the child and family’s life is to improve or maintain child/student’s emotional and behavioral functioning at home and in the community, to help the family access needed resources for the child including obtaining or maintaining third-party insurance or Medical Assistance coverage, and to reduce the risk of out-of-home placement, risk of maltreatment, or the need for protective services.

4.       You will be expected to complete timely documentation and planning including Individual Family Community Support Plans, Progress Notes and case management authorizations.

 

DAY TO DAY ACTIVITIES:

Treatment - Case Managers will provide services to educate and support families to become empowered to better self-advocate and navigate the various systems with more confidence and skill, as well as the following services:

Assessment – for your clients you will complete a functional assessment of the medical, social, educational, and other services and resources the child will need to address their mental health needs.

Service Plan Development – You will be developing an Individual Family Community Support Plan (IFCSP) based on the functional assessment and diagnosis.

 

Dependent on each individual case you will Refer, Coordinate and Monitor Services

Education and experience required:

Due to the unique nature of the work we are considering individuals with:

·       Bachelor’s degree in Social Work

·       1 year of experience with mental health compromised children and families.

·       Valid Driver’s License and access to reliable transportation is required.

·       A successful candidate should be able to demonstrate knowledge of cultural diversity and the role of culture in the therapeutic process

·       Additionally this position requires a flexible, resourceful approach to the work with experience or in-depth knowledge of home and community based services.  


$46,000 - $50,000 a year

Salary Bracket: $46,000 - $50,000 (Salary is based on several factors including but not limited to licensure, work experience and education)

Washburn offers benefits to Full-Time benefit eligible employees comprehensive Dental, Vision, Health, Life and Long-term disability insurance.  Additionally, we offer a 403b retirement plan and a flexible benefit option (FSA). 

 

Vacation:  Paid Vacation accrues at 10 hours of vacation per month - up to 15 days / 120 hours annually.

Sick Leave:  Paid sick leave accrues at 6.68 hours of sick per month - up to 10 days / 80 hours.

Holidays:  There are 2.5 days/20 hours of floating holidays per year in addition to the eight standard holidays.

 

***Please note that Internships, Part-Time (less than 30 hours/week) and Temporary positions are NOT eligible for all of Washburn Center's benefits***
